 Died in Friendly Fire Incident, accidentally killed in Southern Angola during Operation Vastrap. During a brief contact the platoon commander had lost his pistol, and when some of the troops returned later to look for it, they walked straight into one of their own stopper groups that did not recognise them and opened fire
